VSB consent forms are now in digital format.  Please visit:  https://myforms.vsb.bc.ca and enter your child's PEN and birthdate to give consent (or decline) each form.

Parents/caregivers can revoke their consent at any time.  You can find your child's PEN in their MyEducation BC account.

Need help finding your PEN?  Visit https://kchoice.vsb.bc.ca/penrquest.aspx and have the following information available:

• Student's Legal Last Name
• Student Birthdate
• Parent/Guardian
• Email Address on file
